A Chronic Disease Management (CDM) Plan is a referral that is written by your GP which provides eligible patients with Medicare-subsidised allied health appointments, like physiotherapy, osteopathy and podiatry.
A CDM Plan may also be referred to as a GP Management Plan (GPMP) or Enhanced Primary Care (EPC) Plan.
Who is eligible?
To be eligible for a CDM plan, a patient must have a chronic medical condition that requires multi-disciplinary/complex care needs.
As per Medicare requirements, your GP will need to assess your eligibility for a CDM plan.
